If you’re a new patient and haven’t received your patient portal access email to create your account, please call our office at (863) 709-1941. We’ll email you initial access to your OrthoFi portal so you can create your account and complete your new patient forms online.
If you’ve already set up your OrthoFi account, just log in to your patient portal to complete your new patient forms.
